I have a big doubt regarding one of the questions of TEM in you book

It's say : a pilot who is running well behind schedule for the day, forgers to turn on the anti icing system and consequently suffer airframe icing. The situation demonstrates.

A latent threat leading to a handling error
A latent threat leading to a communication error
An internal threat leading to a handling error
An anticipated threat leading to a procedural error

Your book say that the correct answer is B WHY it will be a communication error? I belive the answer is A

let me know please because I had this question on the exam and I fail.

This is an errata that was corrected a fair while ago. You are probably using an older edition. The answer should be A - the latent threat is the organizational time table pressure and the handling error is not activating the anti-ice system.

This is a communication error
A procedural error
A handling error?

Without seeing the actual wording of the question, I would say, since the pilot and captain were communicating, it would be a communication error.

Is really tricky bc I thought communication but during and emergency you have procedures and he reed wrong the speed.

There was a question about a pilot that he just take off with the cover on the static tube. The speed what wrong but the pilot didn't realize about the speed problem.
Where was the error?

In the speed indication
In the cover of the static tube
Or failing to make a good pre flight check to the airplane..

I agree. In the TEM model the error is something the crew did or failed to do. The erroneous speed indication and the static port cover are not the errors. However, the crew failing to remove the cover is indeed an error. For extra marks, can you say what type of error it was? Handling, Communication or Procedural?
